Through its Comprehensive Senior Assessment Services, specially trained staff members at Trinitas Regional Medical Center evaluate each patient’s functional ability, physical health, cognitive and mental health, and socioenvironmental situation. Then, Trinitas Regional Medical Center develops a highly personalized plan of care for each patient, including home health care, day care, and care management services. Counseling services on Medicare, Medicaid and supplemental health insurance are also provided. Through this program, Trinitas Regional Medical Center is able to assess and manage each patient with an integrated approach, ensuring that all of their health care needs are met.
